 Certificate Request Forms 

When you have satisfactorily completed all of the necessary course work with a grade of "C" or better, please submit a Certificate Request Form.  Each Certificate has its own Request Form. 

CEUs

The Continuing Professional Education Unit is a nationally recognized unit of measure used to accumulate a standardized permanent record of an individual’s participation in noncredit continuing professional education programs conducted under responsible sponsorship, capable direction, and qualified instruction.  Units are awarded based on a standard of 1 unit per ten hours of instructional contact.

Class Size

A maximum class size is established for all courses.  Class size is determined by the facility resources.  “Overbooking” is not possible.  Please register early to assure a space in the course/s you want.  We do not maintain waiting lists.

Computer Access

A computer access account has a User ID and password to provide security for both the student and the University in using PSGV computers.  The User ID is different from the PSU ID.  

New Computer Technology Students:  An Access Account will be set up for you 3 days prior to your class start date (providing you have registered at least 3 days in advance), and will remain active for the duration of your class.  You must visit the Computer Center (lower level of the Main building) in person, with a photo ID, to receive your User ID and password.  You have the option of changing the PSU assigned password.  If you have not activated your account before the first class, you will not be able to access any PSGV computers.

Returning Computer Technology Students:   Your Access Account will be reactivated (with your previous User ID and the original PSU assigned password) prior to your class start date.  If you have forgotten your User ID and/or originally assigned password, you need to visit the Computer Center in person within 3 days of your class start date. 

Course Cancellation

The University reserves the right to cancel classes because of insufficient enrollment or other unforeseen circumstances.  Full refunds will be given in these events.  In such an event, you will be notified prior to the beginning of class either by telephone, mail, or e-mail.  It is important we have up-to-date addresses and phone numbers to contact you.

Course Substitutions

You may substitute a higher level course for a required course if you are already competent in an area.  You may also substitute a higher level course for a required course if you have already applied a required course to another certificate.  Contact the Program Manager for pre-approval.  A maximum of two courses may be substituted in a certificate program.

Grades

An official grade report will be sent to you from University Park approximately two weeks after classes end.  It is against University policy to release grades by telephone, fax, or e-mail. 

A grade is given solely on the basis of the instructor’s judgment  A grade A,B,C,D,F or a Pass/Fail (P/F) may be given.  Successful completion is a grade of "C" or better.  An Incomplete (I) will be given for uncompleted course work.  A grade will be awarded upon completion of the course work within a time frame designated by the instructor, not to exceed one year. 

Prerequisites

Certificate Programs:   If a course is not listed as a “required” course, it is NOT applicable towards a certificate.  Please check with the Continuing Professional Education Office if you have any questions about prerequisites.

Individual Courses:   You must complete all prerequisites required in the course description before taking a course.  You may meet requirements either through course work or work experience.  Instructors can evaluate work experience.  Contact the Program Manager if you have a question. Please review our Withdrawal/Refund policy.  No accommodations will be made if you attempt to take courses without complying with the prerequisite requirements.

Textbooks

Textbooks are available from the PSGV Bookstore.  Please wait until the first evening of class to receive textbook information from your professor. 

The PSGV Bookstore will give a full refund up to 7 days after the class has begun if you have a receipt and the book is in original condition. They also have a ‘book buy-back’ option.  Whenever possible, they also offer used books for sale. 

The bookstore will take textbook reservations and will send your textbook to you by UPS for a service charge, plus the cost of postage.  Currently enrolled students can purchase software packages at the PSGV Bookstore at academic prices that are up to 75% off competitor's prices.

Transcripts

Official transcripts must be ordered from University Park with a Transcript Request form.  There is no other way to obtain an official transcript.  PSGV administrators are not allowed to request a transcript on your behalf.
