After you complete your course work in a Penn State Great Valley Continuing Professional Education program, fill out a Certificate Request Form to receive your Program Certificate. Each program has its own request form. You may print out a copy of the request form for your program from the list below.
After completing the form, drop it off at the Continuing Professional Education
office, located on the 1st floor of the Main Building, or mail it to:

If you have lost a certificate, replacement certificates are available for a $25 fee.
